Financial Assistance: Rs. 1,000 per month

  1. The applicant should be a permanent resident of Uttar Pradesh state.
  2. The applicant's husband has died.
  3. The age of the applicant should not be less than 18 years.
  4. The annual income of the applicant and her family from all sources should not exceed 2,00,000/- Exclusions The applicant should not be receiving pension from any other scheme of the state or central government
  1. Colored passport size photo.
  2. Age-related certificate (educational records/provided by medical officer).
  3. Income Certificate (provided by Tehsil).
  4. Death certificate of husband (local body/gram panchayat).
  5. Aadhaar Card/Aadhaar Registration Number (No need to submit separate certificate for age and residence).
  6. Photocopy of bank account with IFS code.
  7. Mobile number.
  8. Not receiving pension from any other scheme of the state or central government related affidavit.
